Development of Integrated Care Teams
Challenge
The local health and care system had a projected financial deficit in the long-term. Commissioning arrangements needed to pivot to focus on patient safety, quality of care and proactive care, focussing on outcomes. A new model of care was needed that involved the development of a network of integrated care teams.
Our work
We supported a clinical commissioning group in making a direct impact in aligning the primary care interface with community and social care through driving up work in integration of health and care services by operationally managing integrated care teams across a geographical footprint of 100,000 registered patient population. This included working with multiple stakeholders including local and unitary authorities, community mental health trusts and primary care practices as well as a range of voluntary care sector organisations in developing into integrated care teams
Outcome
We transformed the delivery of integrated care using a patient-centred approach to care, allowing care to be more proactive.
