Leading Population Health Management Capabilities
Challenge
Finding the art of the possible and pioneering the use of Population Health Management capabilities, allowing the system readiness and development for population health management.
Our work
We worked with an Integrated Care System in the South of England supporting health and care organisations in the reduction of health and care inequalities by leading the deployment of population health management capabilities and big data. The aim of using an intelligence system in the planning, commissioning and delivery of integrated and collaborative care across the system was delivered by our expert team who developed the strategic plan for population health capabilities for the integrated care system, focussing on frontline digitisation and garnering further support for the use of data and intelligence and the role of wider determinants of health to focus on proactive care and integrated of health care for the population.
Outcome
A series of communications and engagement campaign was undertaken to build an appetite for the pioneering work. Our team worked with IT and clinical systems specialists to understand the current technical architecture and develop the technical specification for the software capacity that was needed.
